Instructions:

(1) Select a form name from the list below to open the form.

(2) Fill in the form fields.

(3) Download the form to your computer, email to BSC, or print and mail a copy to BSC. Do not send any National forms to the National office. BSC will record the forms and forward them to National.

VVA Election Report – Note: All Ohio chapters shall submit election results to the BSC no later than July 1st of the year in which the election takes place. (VVA Constitution: Article II: Section 13 and Article III: Section 9). The BSC will record the reports and then forward all election results to the National Membership Dept. DO NOT send Election Reports directly to National. Direct questions to 1st VP Gene Wilt.
